NEW WARNING
The FDA has issued an alert about the use of codeine-containing products by breast-feeding mothers. Breast-fed infants whose mothers are using codeine-containing products are at risk for serious morphine overdose. Mothers who are breast-feeding and who use codeine-containing products should be told to monitor the infant for signs of morphine overdose (increased sleepiness, difficulty nursing, difficulty breathing, limpness) and to seek medical help immediately if these occur. If codeine is prescribed to a breast-feeding mother, the smallest possible effective dose for the shortest period of time should be used. Those most at risk are infants breast-fed by women who are ultra-rapid metabolizers (CYP2D6 genotypes). There's an FDA genetic test approved to detect rapid metabolizers, but there's limited data on its ability to detect rapid metabolizers of codeine.
