NEW INDICATION & DOSAGE
Treatment of acute ST-segment elevation MI, managed medically or with subsequent percutaneous coronary intervention--
Adults less than age 75: 30 mg I.V. bolus plus 1 mg/kg subcutaneous dose followed by 1 mg/kg subcutaneously every 12 hours with aspirin.
Adults age 75 or older: 0.75 mg/kg subcutaneously every 12 hours.
ADJUST-A-DOSE:
Patients with renal impairment who are younger than age 75 should receive a 30 mg I.V. bolus followed by 1 mg/kg subcutaneous injection, then 1 mg/kg/day by subcutaneous injection. Patients with renal impairment who are older than age 75 should receive 1 mg/kg/day subcutaneously.
