NEW INDICATION & DOSAGE
Extended treatment of symptomatic venous thromboembolism (VTE), to reduce recurrence of VTE in patients with cancer--
Adults: Month 1—200 international units/kg/day subcutaneously; total daily dose shouldn’t exceed 18,000 international units. Months 2 to 6—150 international units/kg/day subcutaneously; total daily dose shouldn’t exceed 18,000 international units.
ADJUST-A-DOSE:
For patients with thrombocytopenia, daily dose should be reduced by 2,500 international units until their platelet count recovers to greater than 100,000/mm3. For patients with renal insufficiency, when creatinine clearance is equal to or less than 30 ml/minute, monitor heparin anti-Xa assay with a target range of 0.5 to 1.5 international units/ml; draw samples 4 to 6 hours after Fragmin dosing and only after patient has received 3 to 4 doses.
