AVAILABLE FORMS
Tablets: 1 mg, 2 mg, 4 mg, 6 mg, 8 mg, 10 mg, 12 mg

INDICATIONS AND DOSAGES
Schizophrenia—
Adults: Initially, 1 mg P.O. b.i.d. Increase dose daily, as needed, according to the following schedule: 2 mg P.O. on day two; 4 mg P.O. b.i.d. on day three; 6 mg P.O. b.i.d. on day four; 8 mg P.O. b.i.d. on day five; 10 mg P.O. b.i.d. on day six; 12 mg P.O. b.i.d. on day seven. Maximum dose, 12 mg P.O. b.i.d.
ADJUST-A-DOSE: For patients taking CYP2D6 inhibitors (fluoxetine, paroxetine) and CYP3A4 inhibitors (ketoconazole, clarithromycin), reduce the dose by one-half.

CONTRAINDICATIONS AND CAUTIONS
Contraindicated in patients hypersensitive to the drug or its components. Avoid use with other drugs that are known to prolong QT interval and in elderly patients with dementia-related psychosis.
   Use cautiously in those with a history of stroke, transient ischemic attack, arrhythmias, prolonged QT interval, diabetes, seizures, orthostatic hypotension, neuroleptic malignant syndrome, tardive dyskinesia, leukopenia, neutropenia, agranulocytosis, suicidal ideation, and priapism.

INTERACTIONS
Drug-drug. CYP3A4 or CYP2D6 inhibitors (ketoconazole, clarithromycin, fluoxetine, paroxetine): May increase iloperidone levels. Reduce dose by one-half.
Centrally acting drugs: May increase CNS effects. Use together cautiously.
Alpha1 blockers: May enhance antihypertensive effects. Use together cautiously.
Drugs that prolong QT interval: May cause lethal arrhythmias. Avoid use together.
Dextromethorphan: May increase dextromethorphan levels. Avoid use together.
Drug-lifestyle. Alcohol: May increase CNS effects. Advise patient to avoid alcohol.

ADVERSE REACTIONS
CNS: aggression, delusion, dizziness, extrapyramidal effects, fatigue, lethargy, restlessness, somnolence, tremor.
CV: hypotension, orthostatic hypotension, palpitations, tachycardia.
EENT: blurred vision, conjunctivitis, dry mouth, nasal congestion, nasopharyngitis.
GI: abdominal discomfort, diarrhea, nausea.
GU: ejaculation failure, erectile dysfunction, urinary incontinence.
Metabolic: weight gain, weight loss.
Musculoskeletal: arthralgia, muscle spasm, musculoskeletal stiffness, myalgia.
Respiratory: dyspnea, upper respiratory tract infection.
Skin: rash.

Reactions may be common, uncommon, life-threatening, or
COMMON AND LIFE-THREATENING.
